Contractor agrees to provide to the Department of Mental Health, Coalinga State Hospital, services as described herein.
These services are to provide comprehensive pre-employment physical exams for the Department of Mental Health, Coalinga State Hospital, employees, potential employees, staff, and other persons as ordered by the Coalinga State Hospital physician.
Contractor must provide evidence of qualifications as a California service provider by submitting a copy of their license to practice medicine as issued by the Medical Board of California.
